The group is divided into teams and given various survival tasks. However, their fake "apocalypse" game turns into a real-life nightmare when they find themselves being systematically hunted down by a family of cannibalistic, inbred mutants: the returning fan-favorite Three Finger, alongside his brother and the savage Papa. As the contestants are picked off one by one in increasingly creative and gory ways, a small group, led by the resourceful Nina (Erica Leerhsen) and the athlete Jake (Texas Battle), must battle their way through a gauntlet of booby traps and fight for their lives against their cannibalistic predators, culminating in a bloody final showdown at the cannibals' hideout.

Hosted by a hardcore ex-Marine Colonel Dale Murphy (Henry Rollins), six contestants enter the remote forests of West Virginia to film a post-apocalyptic simulation show. The participants include a stereotypical cast ranging from a goth girl (Nina) to a jock (Jake) and a diva. The horror begins when a contestant named Kimberly takes a detour and is gruesomely murdered by the cannibalistic "Three-Finger" after a wrong turn.
