The film begins with the most spectacular disaster in the series up to that point. Sam Lawton (Nicholas D'Agosto), a young sales representative on a company retreat, has a terrifying premonition that the North Bay Bridge will collapse while his bus is crossing it. In his vision, he sees himself and nearly all of his coworkers perish in a horrifying rain of concrete, steel, and fire.

The plot follows Sam Lawton (Nicholas D'Agosto) and his co-workers from a paper company. While on a bus heading to a corporate retreat, Sam has a violent premonition that the suspension bridge they are crossing will collapse, resulting in a horrific mass casualty event. He manages to get several people off the bridge just before his vision becomes reality. However, as fans know, cheating Death comes with a price.
